School & Child Care Immunization Requirements
Overview
The State of Oregon requires vaccines for school and child care attendance in order to protect children and prevent the spread of disease.
How to get an appointment?
1. Number to call to make an appointment: 541-967-3888 opt 7
OR
2. Registrar online;
Appointment times only:
Tuesdays and Thursdays 9:00 AM - 4:00 PM
- no appointment during 12:00 PM - 1:00 PM
Get ready for your visit:
- Bring insurance card(s)
- Bring any vaccine records if you have them
- Let us know if you need help requesting immunization records from other states
- Allow 1 hour for the appointment
Linn County Health Department currently has these vaccines below for all child ages 6 months and older.
- Covid
- Flu*
- DTaP and Tdap
- Haemophilus influenza type b (Hib)
- Hepatitis A
- Hepatitis B
- Human Papillomavirus (HPV)
- Meningococcal
- MMR
- Pneumococcal
- Polio
- Shingles
- Varicella (chickenpox)*
*Linn County Health Department cannot supply these vaccines to privately insured kids at this time:
- MMRV
- Varicella
- Flu
Privately insured kids that need MMRV, Varicella, Flu can go to a Primary Care Provider or a local pharmacy to make appointment to get these done. Please call ahead to below pharmacies to ensure availability.
