How to get an appointment?

We will call you to schedule the appointment after all the information has been submitted.

 

Appointment times only:

Tuesdays and Thursdays 9:00 AM - 4:00 PM

 - no appointment during 12:00 PM - 1:00 PM

 

Get ready for your visit:

  • Bring insurance card(s)
  • Bring any vaccine records if you have them
  • Let us know if you need help requesting immunization records from other states
  • Allow 1 hour for the appointment

Overview

Immunization is the safest and most effective public health tool available for preventing disease and death. Thanks to vaccinations, we have not seen or experienced many of the infectious diseases that gripped past generations such as polio, measles, rubella, diphtheria and tetanus.

Child & Adolescent Immunization Schedules

7-18 Years

Where to Get Vaccinated

  • Linn County Health Department is providing vaccines for those who are eligible.
  • Contact your primary care provider to schedule a vaccination visit. 
  • Call 211 or 1-866-698-6155 to find a clinic near you--open 6 am – 7 pm daily, including holidays. Languages spoken: English and Spanish. Free interpretation available for all other languages.

  • Go to a local pharmacy, call ahead:

 

How to get your immunization records:

  1. Contact your child’s medical provider. They can print a copy of your child’s record for you.
  2. Talk to the staff of your child’s school or daycare center. If you need a record for childcare or school attendance, the staff may be able to print a Certificate of Immunization Status form from the state immunization information system that will meet the requirement for an immunization record.
  3. We can mail your records to you, please call 541 -967- 3888 option 7
  4. You may stop by our office and request copies of vaccine records in person. Our address is 315 4th Ave SW Albany Oregon 97321
  5. If you prefer to receive your record via secure email, please email alertiis@odhsoha.oregon.gov. Put the desired recipient email in the email body or in a white space on the Record Request form.
