Community Health Improvement Process

The Community Health Improvement Process allows the opportunity for our community to work together to improve health for all in Linn County. The process includes the development of a community health assessment (CHA) and a community health improvement plan (CHIP). As an accredited Public Health agency, Linn County Public Health, in partnership with the community, conducts a CHA and a CHIP  every 5 years. Community Health Assessment (CHA)

The CHA provides an opportunity for all to learn about the health of our community and understand the issues that influence our health. The CHA data helps to identify opportunities for improvement, and discuss what we, as a community, can do to improve our health. The 2022 Linn County Community Health Assessments can be found in the documents below. 

Community Health Improvement Plan (CHIP) 

A CHIP is a long-term, systematic effort to address public health problems based on the results of a community health assessment (CHA) and a community health improvement process. CHIPs help to provide support for developing new policies and determining health promotion strategies. It should define a shared vision for community health through collaboration with diverse partners and address the broad range of strengths, weaknesses, challenges, and opportunities that exist within the community to improve health. 

2022-2026 Linn County Community Health Improvement Plan 

The Partnership for Community Health includes Linn, Benton, and Lincoln county health departments, Confederated Tribes of Siletz Indians, Intercommunity Health Network Coordinated Care Organization (IHN-CCO), Samaritan Health Services, and United Way of Linn, Benton, and Lincoln Counties.

2018- 2022 Linn County Community Health Improvement Plan 

The priority areas of the 2018-2022 Linn County CHIP are: 

  • Community Resiliency: This priority area and community driven workgroup focuses on mental health, substance use, and community well-being
  • Healthy Neighborhoods: This priority area and community drive workgroup focuses on housing, food access, and transportation and building community connection through equitable access to community goods.
  • Reproductive and Sexual Health: This priority area and community driven workgroup focuses on decreasing barriers and increasing access to effective and culturally appropriate reproductive and sexual health information, services, and community resources.
