Wraparound Program
Wraparound is an intensive and collaborative planning process for families with youth experiencing high levels of mental health distress. The Wraparound program emphasizes youth and family voice and choice to identify goals and to coordinate the systems involved with the family. Together, they support the youth and family with working towards their goals and their vision for the future.
Services:
- Comprehensive care coordination services through an interactive process for youth and families with complex mental health needs.
- Eligibility criteria must be met to access this service including:
- Must be under age 21 (0-20)
- Are involved in mental health services and at least one other youth serving system (ex: foster care, special education, juvenile justice, IDD services, etc.)
- Currently have OHP (IHN or Open Card)
- Review and approval for Wraparound by the Local Care Coordination Committee
- Guardian involvement is required.
