To Access Services:
To self-refer, call (541) 497-6205 and request a screening. The screening is used to determine if our Program is appropriate to address the presenting concerns.
Community partners, such as schools, Department of Human Services (DHS), or primary care physicians, may submit referrals to mhreferrals@linncountyhealth.org.
You may also apply for services by filling out our HIPAA form. Someone will reach out to follow up with you.
