Home visits and care coordination for special health needs.
CaCoon (CAreCOOrdinatioN) is a nurse home visiting program for children with special health needs, from birth through age 21. Through CaCoon, nurses help families coordinate care for their children and get the most out of the services available to them.
It’s worth it! Children served in the CaCoon program make fewer visits to the emergency department. They also have higher rates of immunization, annual well-child, and dental care visits.
The most vulnerable families, such as those with a newly diagnosed infant, are prioritized for services.
CaCoon nurses partner with families to:
- Identify problems and help find solutions
- Identify the child’s strengths and needs
- Connect with healthcare and other community supports
- Make sure the child’s health team works well together
- Help gather information to make health-related decisions for the child

Apply for Services
Children and young adults (age 0–21) who have special health care needs are eligible for CaCoon. Children qualify for a variety of reasons, and no family is alike. Please contact us to learn how we can support you.
Also, note—anyone can request CaCoon services! Referrals come from families, medical providers, educators, or other community service providers. If you know someone who may benefit from CaCoon, you are welcome to inquire on their behalf.
