Community First Choice State Plan (K)
Community First Choice State Plan (K-Plan) allows Oregon to provide home and community-based services and supports while receiving a 6% increase in federal medical assistance funds from the federal government for those services.
The services provided benefit Oregonians who want to stay in their home and remain independent, healthy, and safe. At the same time, it saves both the state and federal government money because we are providing more extensive supports and services in a less restrictive environment meaning in lieu of more expensive institutional-type care.
An individual must be receiving services through Linn County Intellectual/Developmental Disability Program and be on the Oregon Health Plan in order to receive this service. Individuals who are not currently on the Oregon Health Plan may be eligible for K-Plan services based on Presumptive Medical Eligibility; i.e. based on individual’s income and resources only.

- Home Delivered Meals

- Assistive Devices

- Emergency Response Systems
- Environmental Modifications
- Community Transportation-Non Medical
Brokerage Support Services
Brokerage Support Services are in-home or other personal supports that assist an individual to live in their own home or with family or friends and to fully participate in community life, including work. To obtain support services, individuals are first placed on a wait list and then referred and enrolled in a Support Services Brokerage by the individuals Service Coordinator based on an Order of Enrollment. Each individual will receive an individual plan and will be able to select and monitor the providers of desired services. The primary job of a Support Service Brokerage is to help an individual plan, arrange, and monitor supports needed to stay at home and use the community.
