Support for Developmental Disabilities
Every individual enrolled in services is assigned a Services Coordinator.
For younger children, our Service Coordinators work with families to assure that they have access to all available community and state resources. They assist the family to advocate for the child’s needs in school and other areas.
For older children, we will work with the individual, family and school to begin planning the individual’s future and ensuring a school situation that will support those plans. The Service Coordinator will assist the individual with the transition from school services into adult services.
For adults, your Service Coordinator will assist the individual with connecting to housing services, in-home services, vocational services, and community resources.

A Services Coordinator provides case management services and can assist with finding resources and supports to meet the needs of our consumers on an individualized basis. Some of the the case management we provide includes:
- Networking and outreach
- Facilitate Individual Support Plan (ISP) meetings
- Assist in the referral process to community service agencies for supports
- Provide support to individuals and families in a variety of planning meetings
- Individualized Education Plan (IEP) support
- Needs assessments
- Vocational planning and referral
- Safety Planning, including assistance with LEDS enrollment
- Assist in finding residential placement and referral
- Make referrals to support services brokerages
