Healthy Communities
Health begins where we live, learn, work and play. Our Healthy Communities program works to create opportunities for everyone to be physically active, eat nutritiously, avoid tobacco, and experience fewer chronic conditions. Our goal is to create communities where we all have access to healthy options.
One of the ways we do this is through policy, system and environmental changes. Why? Where you live can affect how well you live, and how long you live, too. Policies, systems, and environments influence the way people behave. At their best, they can make healthy living easier for everyone.
Projects & Priorities
Community Health Improvement Process
We lead the development of the Linn County Community Health Assessment and Linn County Community Health Improvement Plan.
Increase Access to Healthier Foods
We partner with Samaritan Lebanon Community Hospital to enhance the nutrition environment of the hospital’s dining area. This has included the implementation of a sugar sweetened beverage reduction campaign and a sodium reduction project.
Increase Access to Physical Activity
Connecting the Dots between Health and Transportation in the Linn-Benton Region brought together 100 community members and leaders from Linn and Benton Counties to learn about, discuss, and lay a path toward building healthy communities.
Prevent Chronic Disease
We collaborate with Benton and Lincoln County Health Departments, InterCommunity Health Network CCO (IHNCCO) and local health care providers to implement The Colorectal Cancer Screening Campaign.
Manage Chronic Disease
We work with Samaritan Health Services and community partners to increase the availability of chronic disease self-management classes for all in Linn County.

How Do We Improve Community Health?
Through our Community Health Improvement Process, we invite our community to come together on a shared vision of community health.
This starts with a community health assessment (CHA). Reading the CHA, we learn more about the current health of our community, the issues at play, and what we can do to improve it.
Out of this comes our community health improvement plan (CHIP). This is a long-term, systematic effort to make that happen. CHIPs support the development of new policies and health promotion strategies. They define a shared vision for community health through collaboration with many different partners. They address a broad range of strengths, weaknesses, challenges, and opportunities.
As an accredited Public Health agency, our public health program team works with the community to conduct a CHA and a CHIP every five years.
The Partnership for Community Health includes Linn, Benton, and Lincoln county health departments, Confederated Tribes of Siletz Indians, Intercommunity Health Network Coordinated Care Organization (IHN-CCO), Samaritan Health Services, and United Way of Linn, Benton, and Lincoln Counties.

Community Resiliency
Our plan identifies ways we can work together toward improvements in mental health, substance use, and community well-being.
Healthy Neighborhoods
We also focus on housing, food access, and transportation, building community connection through equitable access to community goods.
Reproductive and Sexual Health
We want everyone to have access to effective, culturally appropriate information and resources for reproductive and sexual health.
